Vision (Retiree)


This vision program is for PEF retirees only, and available beginning November 1, 2017.

Healthy eyes and clear vision are an important part of your overall health and quality of life. The Davis Vision benefit program helps you care for your eyes while saving you money.

The Davis Vision benefit is available to dues-paying members of the PEF Retirees and their families. This benefit is not included in your dues payment. You must purchase the benefit separately.

Advantages of Enrolling in the Vision Benefit:

  • Eye Exam–a comprehensive eye exam at minimal out-of-pocket cost to you
  • Free Frames*–Select a Davis Vision Exclusive Collection frame from nearly 10,000 independent, participating providers at little-to-no out-of-pocket cost. OR, choose any frame at an in-network provider and use your frame allowance.
  • Fixed Lens Pricing–selection of the latest lens technology available with set pricing on all lens options.
    *Fashion level frames in the Exclusive Collection are free.


All active, due-paying PEF Retirees are eligible to participate in this benefit.

Renewal notices are mailed to existing program participants.


The plan year runs from November 1, 2017–October 31, 2018 regardless of when you enroll during this time frame. Benefit must be renewed/re-purchased if you would like to continue taking advantage of the product and services discounts for the 2018/2019 benefit year. Renewing /re-purchasing the benefit is optional.  It is not mandatory that you renew this plan yearly if you are not interested in the annual product/service discounts offered.

How to Enroll:
1. If you are a current PEF Retiree
2. Once you purchase your vision benefit for the plan year of November 1, 2017–October 31, 2018, you will receive a confirmation notice regarding your purchase, as well as, information regarding the next steps of the enrollment process, including:

  • 72 hours after you purchase the benefit, you need to complete the enrollment process directly with Davis Vision.
  • Contact Davis Vision at 1.844.681.4498 and reference Client Code: 2395.
  • Provide Davis Vision with your, Membership Identification Number (MIN), name, and date of birth.
  • If coverage is purchased for your spouse or your family members, provide the name(s) and date(s) of birth for each family member.
  • Once you have completed your enrollment process, you will receive a vision packet including: plan information, ID cards, and a provider listing for your area. Thereafter, to access your benefits, you will need to provide your PEF Membership Identification Number (MIN) located on your PEF Retiree ID Card, when calling Davis Vision directly or making an appointment with the eye care provider.

Not a current PEF Retiree?

If you are not a current member of the PEF Retirees, please complete and return the PEF Retiree New Enrollment Form or the PEF Retirees Renewal Card. If you are choosing to pay by pension deduction, you will also need to complete and return the pension deduction form.

First year membership is FREE (if you retired after April 2006). Year two and thereafter, membership dues are only $18 annually via automatic pension deduction, or $21 annually when paying by check.

If you retired prior to April 2006, membership dues are only $18 for the first year and every subsequent year via automatic pension deduction, or $21 annually if paying by check.

Forms must be received and processed by the PEF Retirees prior to your purchasing the Davis Vision plan. Once your enrollment form and payment has been processed, you will receive a PEF Retiree Membership Identification card.
